Disease Management

Helping participants self-manage themselves

The Disease Management Program is designed to help participants better manage the treatment of their chronic illness and maintain optimal health.  The management of Diabetes, Coronary Artery Disease, Heart Failure, Asthma, Chronic Obstructive Pulmonary Disease (COPD) and Chronic Low Back Pain are the focus of the Disease Management Program.
 
A specially trained health coach provides regularly scheduled phone calls, at your convenience, to help you achieve your health goals.
 
After walking you through an initial assessment, your health coach will explain the risk factors of your condition and what you can do about them.
 
Your health coach will provide you with methods in self-management to encourage positive health outcomes, and help you specifically with a personalized plan including:
  • Compliance with medication
  • Compliance with treatment
  • Complication prevention
You will receive information regarding the disease process and a customized action plan.

This program is free to all MPI active eligible participants.

Nancy Fisher
, RN, is experienced in medical and neonatal ICU, utilization management and appeals and grievances with an HMO and as an occupational health nurse for CBS TV.

Christina (“Tina”) Hurst
, RRT, is experienced in neonatal pediatric respiratory care with home health care experience.  She recently helped establish an asthma disease management program for Blue Cross, and has more than 18 years of clinical experience.

Kathy Touake
, RN, BSN, CCM, is experienced in health coaching for Blue Cross, with six years case management (CM) for a state sponsored program at Wellpoint with certification in CM.   She also has 18 years experience in labor and delivery, postpartum, and nursery.  She has taught prepared childbirth classes for 13 years at St. John’s Regional Medical Center and has worked in oncology and home health as well.  In addition, she served as an Air Force Nurse for three years.
 
Richard Peng, RCEP and HFI through the American College of Sports Medicine, MS in Exercise Physiology, is experienced in health coaching for Blue Cross (WellPoint) and health education, cardiac rehabilitation and diagnostic testing, and adult fitness.  He does peer review for American Association of Diabetes Educators.  He has taught CHF, Low Back Pain, and Weight Management classes.

Leslie Slate, RN - experienced in inpatient and outpatient behavioral health.  Has a Masters Degree in Public Health, Health Education.  Also has experience in non-profit community health agency working with refugees.

Paul Baker - Senior Intake Specialist - Has had 8 years in the medical health information industry ranging from psychiatry to ophthalmology.
