MPTF's Disease Management Program is designed to help participants better manage the treatment of their chronic illness and maintain optimal health. Our program focuses on the management of Diabetes,
Coronary Artery Disease, Heart Failure, Asthma, Chronic Obstructive Pulmonary Disease (COPD) and Chronic Low Back Pain.
A specially trained health coach provides
regularly scheduled phone calls, at your convenience, to help you
achieve your health goals.
Education
After walking you through an initial
assessment, your health coach will explain the risk factors of your
condition and what you can do about them.
Self-Management
Your health coach will provide you with
methods in self-management to encourage positive health outcomes, and
help you specifically with a personalized plan, including compliance
with medication, treatment and complication prevention. You will
receive information regarding the disease process and a customized
action plan.
Nancy Fisher is a Registered Nurse (RN) and a Certified Diabetes Educator (CDE). She is a personal health coach for the Motion Picture and Television Fund’s (MPTF)
Disease Management Program and teaches diabetes self-management classes through MPTF’s Wellness Program. She is a member of the American Association of Diabetes
Educators and the American Diabetes Association. Prior nursing areas of expertise include medical and neonatal intensive care, utilization management and appeals and
grievances with Health Net, school nursing, and as an occupational health nurse for CBS Television. Her previous work experience as a nurse at CBS Television familiarized
her with entertainment industry client needs.
Christina "Tina" Hurst | RRT
Experience in neonatal pediatric respiratory care with home health care experience. Tina recently helped establish an asthma disease management program for Blue Cross,
and has more than 18 years of clinical experience.
Kathy Touake | RN, BSN, CCM
Experience in health coaching for Blue Cross, with six years case management (CM) for a state sponsored program at WellPoint with certification in CM. Kathy has taught
prepared childbirth classes for 13 years at St. John's Regional Medical Center and has worked in oncology and home health as well. In addition, Kathy has served as an Air
Force Nurse for three years.
Richard Peng | RCEP, HFI & MS in Exercise Physiology
Experience in health coaching for Blue Cross (WellPoint) and health education, cardiac rehabilitation and diagnostic testing, and adult fitness. He does peer
review for the American Association of Diabetes Educators and has taught CHF, Low Back Pain, and Weight Management classes.
Leslie Slate | RN, MS in Public Health, Health Education
Leslie is a Registered Nurse (RN) and has her Master’s degree in Public Health (MPH). She has extensive experience in inpatient and outpatient mental
health nursing having worked at the Thalians Mental Health Center at Cedars Sinai Medical Center. Leslie has worked as a health educator in community-based
non-profit programs for Los Angeles County. Her emphasis has been in cross cultural health, including directing a health education program for Southeast Asian refugees.
Theresa Ranftl | RD
Theresa is a personal health coach for the Motion Picture and Television Fund’s (MPTF) Disease Management Program.
She is a registered dietician having completed her course work at Pepperdine University. She also holds a Bachelor of
Science in Criminal Justice from Cal State Long Beach. Theresa has extensive experience with weight management and
eating disorders. She is a member of the American Dietetic Association and has a Certificate of Training in Adult Weight Management.
Theresa has been at the UCLA Center for Human Nutrition since 2000 where she has been involved with volunteer and administrative work;
clinical research; and with the Risk Factor Obesity Program. She has specialized in eating disorders at the in-patient and out-patient
levels for The Bella Vita, an eating disorder treatment facility. Theresa also has been involved with long term care and adult day care
health facilities, medical foundations and private practice.
