What Is a Community Care Team?

The goal of the Community Care Team (CCT) is to provide high quality multi-disciplinary medical and supportive care for Industry members and their parents in the setting where they live:

  • Skilled Nursing Facilities
  • Rehabilitation Facilities
  • Assisted Living Homes
  • Board and Care Facilities
  • Private Residences

MPTF's Clinical Care Team includes:

  • Physicians
  • Nurse Practitioners
  • Registered Nurses

MPTF Support Services Team includes:

  • Social Workers
  • Pastoral Care
  • Volunteer Staff

Who Will the Community Care Team Serve?

The CCT is based out of the MPTF Age Well Program located at the Saban Center for Health and Wellness in Woodland Hills. CCT services are available to persons residing in select MPTF service areas. These areas continue to expand. Please call for information about current service areas.

Services will include:

Referrals

  • Community Support Services
  • Financial Assistance
  • Home Health
  • Hospice or Palliative Care
  • Psychosocial Support

Consultations

  • Level of Care Assessments
  • Comprehensive Geriatric Assessments
  • Palliative Care

Ongoing Medical Follow-up

  • Office Visits
  • Home Visits
  • Hospital/SNF Visits

What Are the Benefits of CCT Care?

The Community Care Team offers a wide variety of benefits for clients and families:

  • Individualized plan of care
  • Medical care at the MPTF Age Well Program office or at a client's residence
  • Support services to maintain seniors in their residence
  • Home safety assessments
  • Palliative care
  • Resource for hospital Discharge Planners for Rehab or SNF placement of MPTF clients, including assistance with relocation
  • Link to community resources for Industry members and their families
  • Emotional support for caregivers and families
